School Approval and Course Request Form Bank

Overview

As part of the admissions process for Credit Programs at the University of Mississippi, applicants must submit a School Approval and Course Request Form to be approved to participate in dual enrollment programs and/or approve high school credit for college courses taken. This form bank houses the School Approval and Course Request Forms needed for each Credit Program offered by the Office of Pre-College Programs. Please be sure to download the appropriate version of the form for your selected Credit Program.

One School Approval and Course Request Form must be completed for EACH semester or session enrolled. If participating in Summer sessions, one set of forms is needed for First Session and another set for Second Session.
